It opens a real TCP connection to the IP and port you enter, then reads what the remote side sends back. An SSH server identifies itself immediately with a line starting
SSH- — so a pass here means an actual SSH daemon answered, not just that something is listening on the port.
Something accepted the connection, but whatever answered didn't identify itself as SSH within a few seconds. That usually means a different service is using that port, a firewall is accepting the connection without forwarding it anywhere, or the SSH daemon is unusually slow to respond.
It's easy to update sshd's config and the firewall rule but miss one of the two, or reload the wrong service — the port looks “changed” in your config but isn't actually reachable.
