MailWizz is commercial PHP campaign software sold under a one-time licence rather than a subscription. Its distinguishing feature is scale of operation: many lists, several configurable delivery servers with routing between them, and the ability to run the whole thing for multiple customers. That last capability is why hosting companies build sending businesses on it — and why the licence choice matters, because charging customers for access requires the $275 Extended licence rather than the $89 Regular one. Like every campaign application here, it does not deliver mail itself: it hands messages to an SMTP server or a relay you configure.

Regular or Extended — Get This Right First
The difference is not features, it is what you are allowed to do with the installation. Buying the wrong one is an expensive correction.
Regular — $89
For using MailWizz to manage your own subscribers, lists and campaigns. This is the right licence for a business sending its own marketing, however large the list gets. One-time payment, one month of support included.
Extended — $275
Required when you want to charge customers for access to your MailWizz system — that is, when the installation is the product. If your plan is to resell sending, this is the licence, and budgeting for the $89 one would be a mistake.

It is a good product aimed at a specific job. If that is not your job, it is money spent for nothing.
You are running sending for other people
Multi-customer support is the reason MailWizz exists in the form it does. Nothing else here handles it as well.
You need several delivery servers with routing
Splitting volume across relays and your own MTA, with rules about what goes where, is built in rather than bolted on.
You prefer paying once
A one-time licence against a per-subscriber subscription is straightforward maths once a list is large.
You want something mature and documented
It has been around a long time, and the documentation and community reflect that.
